God it's all too hard for most people and maybe, understandably so.
People feel unsure what to say after a while when an unimproved health condition confronts them, but they keep asking; how are you?
It would be best if they just said; Hi, it's good to see you!
kilroy, I've spent 5 years living in a world of chronic pain where I need to manage every aspect of my life. I live my life in 15 minute blocks. That's my pacing time frame. I can sit for 15 minutes, stand for 15 minutes... and so on.
People get sick of me all the time. I'm a human yo-yo.
Never worry about what others are thinking because that's impossible for you to control.
Instead, help people understand what you need, and how to communicate with you.
Maybe start by suggesting they not ask how you are, but instead say it's good to see you. ♥
Oh.. and give your wife some good old fashion lovin'

I hated when a salesperson whom I had never spoken with on the phone, used to have a spiel:
Good morning, my name is .............. how are you today? As if they cared.
My dear old dad used to respond: Are you sitting down? This should take 20 minutes or so. And I wan't you to know how pleased I am that you phoned me to ask after my health.

Unfortunately, our face can reflect our pain (whether physical or mental) and people assume you are a grumpy old *itch or *astard; little knowing our discomfort.
I try to make my grimace look like a smile.
One day, I overheard someone reply to the how are you? with "at least on I'm on the right side of the grass"
To anyone experiencing pain today, I hope it is not too extreme.
