LINDT cafe manager Tori Johnson was forced to kneel on the floor before gunman Man Haron Monis shot him in the back of the head.
Counsel assisting the Coroner, Jeremy Gormly SC confirmed that Johnson had been killed by Monis, while another hostage Katrina Dawson, a 38-year-old lawyer, was killed by six fragments of police bullets that were fired at Monis as they stormed the cafe. One struck a major blood vessel causing her death.
Before the siege began, Monis had eaten a piece of chocolate cake and was drinking tea before asking to move closer to the rear doors of the cafe. He then asked to see Mr Johnson.
Staff watched as he spoke to Mr Johnson, who became visibly stressed as he listened to the gunman's demands. Monis told Johnson to call 000 to say Australia was under attack by Islamic State.
