What happens if a buyer has received a product, left feedback, etc. but doesn't confirm delivery? What does eBay do and how/when do you get paid?

You haven't sold for a long time so eBay will see you as a new seller.

You'll need to be signed up for managed payments and after that, your payment will generally be on hold for 30 days

so what if i add a payment card but my account is still suspended?

If your account is suspended, that is a separate issue.

Adding a card IS NOT signing up to MP, which is what you need to do in order to be paid.
