Missed miscarriages are quite common. 

 

The embryo dies and the woman has an ultrasound not knowing the embryo has died - this is commonly known as a missed misscarriage. She may actually miscarry the pregnancy (with all the obvious signs) up to a few weeks later. If not, a D&C is usually recommended.
