What is the second number below the starting price?

I dont see the $22.91 when I am viewing that listing. So don't know.

 

I also notice that the seller has specified "Cash on Delivery" as a payment option.

But no cost for this is given. So presume its included in the item sale price.

So I wondered how far he would drive to dliver it to me?

 

It's common for sellers to confuse "Cash on Pickup" with "Cash on Delivery".

However, the latter can be costly if a buyer enforces it.

What is the second number below the starting price?

The top price is the price in the currency of the country where the item is listed.  The bottom price is the approximate price in the viewer's currency.  If you look at items on the US eBay site, you wil see the top price in US dollars, and the  approx AUD price below.  But from the looks of that, whoever took that picture is in another country and is looking at the AU site, as the postage and the top prce appear to be in AUD and the price below in a country with a currency rate well below ours.

It looks like an Australian listing being viewed on the US site....the bottom amount is the US$ value of the AU$ listing price.
