The gunman mostly had another hostage near him. There was a younger woman who was physically disabled and he kept her near him holding on to her. She wasn't able to get up and runaway. Her mother was also in the cafe and she stayed by her daughters side.
Other hostages have told of having to stand near him.
The office Manager was killed before the police entered and the woman who died did have some wounds from shots but died of a heart attack.
