Selling multiple items to the one buyer and combining postage

Before a buyer of multiple individually listed items are paid for, how do they pay for the goods without paying postage over and over and what do I need to do as the seller?

In addition to Dave's advice, if the system won't let you send an invoice, it may be easier to create a new listing as a special order combing all the items and with the postage amount you want to charge. The send the new item number to the buyer. When I have to do that I let the buyer know first and do follow up email with the new number.
