If the buyer hasn't paid yet then hopefully they will understand and accept a cancellation.
In future if you are doing a special listing I would leave the description out of the title so that it is not searchable and just have "Listing for xxxx". Then as soon as it is listed send the buyer a link and hope that they buy it first.
I would also setup a buying ID so that you reduce the worry about retaliatory feedback on your selling ID.

You really should have blocked the seller that you left the neutral for.
If the message where they have stated they'll leave you retaliatory feedback has come via ebay,you need to phone ebay NOW & report directly as its against ebay policy to do this.
Solve THAT issue before anything else & explain to your genuine buyer what has happened & that there will be a short delay.
Buyers who do not meet the sellerโs terms as outlined in the item listing are not permitted to bid on or buy the item. Furthermore, buyers are not permitted to place bids or purchase items with the primary intent of disrupting a listing

Another way I've found successful, is to list a really high postage amount ..... $50 or so.
That seems to stop other buyers. I advise the customer that I'm doing this, and to wait for an invoice. Then I adjust the postage back to the right amount, once they've purchased.

To answer some of your points:
The strange buyer has already paid, they paid immediately
The seller that I annoyed some months ago is on my blocked list but this is a different user, and she had advised me that she would use one of her other ID's to do this to me.
I did report this seller months ago to ebay because she did threaten me through ebay messages and personal email, and all of that was sent to ebay. But I can see she is still a seller anyway (and I'm not the first or the last she has threatened). She also made lots of other threats about what she would do to me. With stuck crap feedback I don't even know why ebay didn't shut her down.
i did create the listing and send it to the buyer that it was intended for but she didn't get to it in time I gather. She went to purchase its a few hours later and it was already gone.
I sent an email to the new buyer asking if they had purchased it in error as it was a listing for another ebayer. I still have no response. Should I send a cancellation? My other buyer is understanding but not sure for how long.

Get a commitment on the cancellation through ebay messaging first. Then refund and send the cancellation. If they decline it, ring ebay and explain you have in writing they want to cancel.
If they don't respond to messages, refund and send the cancellation with explanation and it's auto closed after 7 days of non action from buyer.

I can't emphasise strongly enough-RING EBAY.
The fact she's used another id is even worse.Forget the Mutual Cancellation-if you definitely know it's the same vindictive seller,get on the phone NOW.

I was guessing.....wondering if the buyer who had bought this item was the seller I had annoyed some time ago.
turns out it wasn't YAY!!
I have spoken with both buyers and since there are duplicates of the book in the listing they are buying half each, so win win for all.
The buyer that wasn't suppose to buy the item as aid he didn't know what it meant when I put another ID in the listing, he thout I was selling the item for another member. Sounds all a bit strange but it appears to have worked out now. I'm so glad I just waited for a response from buyer.
