Have had a case opened against me for an order that the buyer cancelled himself, what the???

About the only feasible explanation I can think if that could lead to something like that is they paid, funded the payment with their credit card, and thought the refund would appear straight away back to their credit card (they can take a few days to process back), so thought the refund hadn't been issued... (not that a quick message etc wouldn't have sorted that out, if that's even what happened).
If that's not it... ¯\(°_o)/¯
(and even a little if it is, as PayPal would've more than likely sent an email notification - hope it doesn't result in a defect, or that eBay remove it if it does ).

Really goes to prove (yet again) how bad this system is - if a Buyer can open a case for an order they cancelled & never paid for - maybe I'm stupid, but I don't see how that should even be possible !! Am I missing somethng ??

Yes, and a seller can open an unpaid item case against a buyer for a purchase they (the seller) cancelled because they had no stock.
And ebay can insist that the seller is in the right and the buyer ends up with unpaid item strikes.
Unbelievable? Under ebay's rules it makes perfect sense. It's just happened to me (two purchases cancelled by seller and two unpaid item strikes against my account) and ebay says (so far,....m still trying to change their minds) that they cannot be removed because it is not in dispute that I did not pay.
Oh, and ebay sent me two emails confirming the sales were cancelled - the first one telling me to seek a refund if I'd paid, and the second one confirming the refund for the cancelled purchases.
The lunatics are indeed in charge of the asylum.
