Went to buy one item for $23 but when confirmed total order was for four items $499.41

eBay don't add items to orders.
Without any relevant information from you (handy to have) I will hazard a guess that you had items in your cart worth about $476.41.
Please provide some relevant information and what you expect other members to do.

Do you allow anyone else access to your account?
As said, eBay do not randomly 'add items' to a buyer's cart

I am like dave and all I can think of is you may have already have had items in your cart.
Okay, so you paid the order for $499.41?
What I would do in your case (if this is what happened) is write to the sellers of the other 3 items, say you made a mistake, the item was bought by mistake as part of several other items and could they please cancel for you? Do it asap.
You cannot cancel items but a seller can and if you have been polite, many sellers may oblige.
In any case, I think you'd hhave a good chance of at least one or two of them cancelling and refunding so at least you'd save some money.
And empty your cart before you buy anything again. You can have items in a watched list if you want to keep track of them, no need to pop them in a cart.
