Buyer hasn't picked up item in 2 years of purchase, has now contact wanting to pick up item. Item was probably worth $20 & now would be out of date.

The buyer does not have a leg to stand on, as far as I can see.
They are out of time for opening any ebay or paypal claim and they are hardly likely to take you to court over a $20 item.
You can ignore the message or you can reply if you want to, to the effect that the item is no longer available, items need to be collected within X number of months.
It's quite rude of them to expect you to hold it there forever, at their convenience.
But your first move should be to block that buyer from purchasing any of your items now or in the future.
If they don't get the item, they may try to make trouble by buying, claiming defective, getting a refund.

You havent specified what the item is. There are strick rules around uncollected goods, and they vary dependent on the item.
What were the goods?
