Hi as title states, How do I combine shipping if a buyer has already paid for 2 or more different items?

too late - just refund the buyer whatever is appropriate - allowing for fees paid.

I usually just work out the new postage cost and refund the difference. I don't bother to deduct any fees incurred, just happy to have the extra sale.
