I personally use FreeForm2 via Robshelp, which is quite easy to use and allows me to create a customised template rather than use one that many others can also use. You can create a variety of templates, and I also use it to create my listings from my saved templates, then copy/paste the HTML into eBay's online form (without saving in Freeform so that it doesn't create a new template).  http://freeform2.robshelp.com/freeform2.htm

Before that, I did use a few different free, finished templates that you can find. In that case it was just a matter of looking at each site and viewing the available templates, then picking a few that I liked and seeing how they looked with my products. I didn't like anything overly graphic heavy, and definitely nothing that glittered, flashed, moved or sang Smiley Very Happy - but I think most of the ones I ended up using came from here: https://freeauctiondesigns.com/template_category/all/

I also used a couple from here: http://www.isdntek.com/tagbot/templates/shipscript.htm as I had a liking for the clean designs (no images, graphics etc) in neutral colours.

I'll do that after I make my first million on ebay. ๐Ÿ˜„ Having not quite reached that goal yet, the most I'll do at the moment is pay Vistaprint to do my return address labels (and even then, only when there's a significant discount coupon involved. Smiley LOL )

How much do they cost?

You know you can buy eBay label paper so you can print the front and return label?

It varies slightly according to the value of the voucher. One of the last ones I had was for $20 or $30, so I got 420 individual labels for a bit under $8.00 (that was the delivery cost), plus I had previously uploaded my own design / logo (they charge for that, but sometimes give free uploads), so all told it was just shy of 2c per (colour) label, no paper/printing costs of my own. I like them for the branding aspect, and it saves a decent amount of time when I have a lot of packages, but without discounts and so forth, they'd be more around the 10c per label range, which I would never pay.

I had to pay for my last lot of return address labels. They don't seem to be discounting them much or often anymore.

The last decent voucher I received was in conjunction with the Melbourne cup. Sometimes they almost tempt me with the $5 mug and t-shirt deals, but then I remember I have nothing practical to do with them (other than feel self-satisfied if/when I use them myself. Smiley LOL ).

They have got me a bit... I've gotten used to the convenience of the RA labels now and I bit the bullet recently and actually paid for some (50% off 420, so this time it cost me - after delivery costs - $20, a bit under 5c a label. I looked into a dymo thingy, but near as I can figure, they cost a lot more per label). There's probably more cost effective alternatives, but I found printing my own labels to be a bit more time consuming and laborious than handwriting the delivery adress, sticker with the return address on the back.

I don't have the turnover to warrant that kind of investment, LOL. (I send out average of 45 packages a week).
