Having done a little more digging, I have managed to find a way to be able to enter the 'automatically decline offers lower than' value. Whilst not as easy as it used to be (as we know it came up automatically in quick listing before).
At the top right hand corner of an already existing draft, there may or MAY NOT BE a 'Switch to Advanced tool', if it is there click that and it will bring up the new advanced format (it will warn you not to use this unless you are 'experienced'). Wait for all the fields to enter in the item description and then you will both see and be able to enter the option for setting automatic accept or decline values.
If this does not work within a draft (did once and then not for me), just save the draft and then in the sell an item page that comes up, in the top right corner you will see the 'switch to advanced tool'. This should then open up a new page with a list of your drafts that will open in advanced tool with the options available. If you do not have any drafts, then it is the same process. Click 'sell' as normal in top leftish, and it takes you to the sell page with the 'switch to advanced tool' in the top right corner!!!
Hope this helps, and I am glad you asked this question as it motivated me to figure out the answr. So fed up of insulting offers, on one item the 'buyer' offered 5% of the asking price!!

Might need a little more info about what you're seeing when you try to use best offer, because although eBay's screen shot is a little oudated, the process is pretty much as they explain it.
eg On the full Sell Your Item page, this is what it shows in the price section, first screen shot shows best offer while unticked, the second screenshot shows best offer ticked and as you can see, the auto accept / decline fields is supposed to appear as soon as BO is ticked (you then just have to tick each one you want to use, since you don't have to use both).

That doesnt happen in ANY web browser for me.
*This* is what I see whether I use Firefox, Safari or Chrome:
No additional pop up menu appears.
I have tried revising the listing. Nothing.
I have tried revising back to a reg auction and then going back in to revise back to a best offer - nothing.
Ebay's help pages are useless and as you said, outdated.
There are NO options for setting auto decline limits within my ebay profile that I can find anywhere.

Have you tried to click on the little i after Add Best Offer,(that should contain the extra info as it stands

I am interested in the answer to this question also! It seemed to disappear as an option when listing about 6-8 months ago, and assumed it was a permanent change or my browser. The only time I now have the option to set automatice decline values is AFTER my first ridiculous offer on an item. Once one offer is made, then the drop down option is available within the offer history. Ridiculous system if it can only be accessed after an offer.

That's odd - as in, your listing form is in a completely different format than mine, normally fewer options and simplified form is via the quick listing tool, which I know you've mentioned you're not using, so I'm not really certain what's causing the difference (just for the sake of anyone who might find themselves with the quick listing format and no access to certain options, there should be a link at the top of the page to switch to the advanced listing tool).
Do you have any "add or remove options" links in the sub-sections? Sometimes additional options can be enabled that way, though on the format I showed above, best offer and auto settings are there by default and can't be disabled.
Not sure if this will help, but some have said saving a listing to draft and then going back in and editing allows you to access additional options.

I did state above that I have tried this.
There is no other option that comes up when selecting "add best offer".
The other user who described the same problem and that they only get to set auto decline amounts AFTER someone has made a first offer is correct. That is what happens to me.
Sigh.
This is not the only unresolved issue I have with ebay, either. This organisation is replete with examples of staggering ineptitude.

Sorry, but not quite.
You said you tried revising a listing, which I assumed meant the listing was active and you tried editing it. A draft is a listing that hasn't been submitted to go live, and some people (after doing some googling to actually research your problem and try to get a workable solution) said that it worked for them.
The other thing is...
As I mentioned, my sell your item form is very different from yours, and just out of sheer curiosity, I decided to switch to the quick listing form, which looks very similar (not identical) to the format you have, and likewise does not provide the option to include auto accept / decline amounts:
Whether or not you've been stuck with this mode for one reason or another, I do not know, nor do I know how to switch to the advanced format if you don't have the link to do so.
Sadly, I also don't know why eBay are jerks, so I guess I wasn't any help at all.
