Byyer wanted to return and posted the item. The item is on the way. If I issue a refund now will this again take money from my paypal account?
ebay already deducted the full payment from my account.

If the buyer opened a dispute and you accepted the return, the original payment will appear as though it has been refunded, but it is in fact on hold until tracking shows the parcel has been delivered then it is refunded to the buyer. Your don't need refund yourself, the dispute will finalise the refund.

It's better to refund manually, though I always wait until the package has been received so that the contents can be checked.
eBay do provide an option in the returns console to issue a refund at any time, so if you view the case details it should show that the package is on the way, and have an option that says something like "If you prefer, you can refund the buyer now" with a link to do so.
As mentioned above, the funds are in holding, so when the refund is issued those funds are just credited back to the buyer.
