It's generally when the flyleaf had a price on a section of the page and that section was removed with a pair of scissors.

If you mean the dust jacket's flap, it's called price clipping - and it usually only affects a small segment of the dust jacket. It means the book was remaindered in a past life and before it was clipped there was a price there.Sometimes older mass market paperbacks have been hole-punched on the front cover to denote remaindering. These days books that have been remaindered are usually marked by a black texta on the bottom edges of pages.

It could also mean a piece of a page has been physically cut out of a page.
For example, a person writes his/her name in a book. When that person no longer wants the book, he/she cuts the section containing the name from the page in the book before donating the book or giving it away.
