If you know her so well I would ask if she has a Gift List with a store....or a Gift Voucher from a ritzy store that she may not shop in normally.
Or something completely non essential but totally decadent that she would not buy for herself.

Balloon ride for two
weekend away with tickets to a show
winery tour
glamping weekend somewhere or spa resort
dinner out at an amazing restaurant
Something like that.
Depends how much you want to spend too, but sometimes they have really good deals on things like that on the various online sales sites such as Living social, groupon, catch of the day. A couple of years back, I got a week for 2 in Cairns in the deluxe room at a hotel, with airport transfers a half day rainforest tour, breakfast daily & a bottle of champagne all for $400. You can often do much better on weekends away etc
