Customer has requested I invoice and send an additional item that I have from home. The item wasn't listed on eBay for sale but I was talking to him about it privately and he wants me to add it to his item he purchased on eBay and make one payment and one parcel. How do I add the item to an eBay invoice at checkout?

Not very bright to come on a public forum and ask how to practice fee avoidance!
What you should do is list it as a BIN so they can buy it then send a combined invoice, that is the right way to do it rather than just adding the cost where it says + or - and then not hitting recalculate;-)

ds, I assume you want to do the right procedure.
As suggested, list it as a buy it now and message the buyer as soon as you have done so.
Then when you send an invoice, tick all the items to be combined, and press send. Do not click recalculate as that reverts back to the original. You can preview to check it's correct.

ebay do not have the option for you to avoid their fees by adding private items to THEIR invoices.

If it was a private sale as stated then why would anyone want to pay fees for it??
You can send invoices through paypal. Maybe create another invoice through paypal for the total (ebay item and private one) and whan payed mark the item as payed on ebay.

Ebay doesn't let you add additional items to the invoice but it does allow you to add extra charges to the invoice.
Send a revised invoice and you will see a box under the postage charges called "Seller discounts (-) or charges (+):". Just enter the additional charge and type the reason for the additional charge in the "Notes for the Buyer" box so they know.

Add the extra fee and describe in the notes what you have charged for. The buyer will not necessarily see the new total until they go to checkout.
If it was against eBay policy to add extras to the invoice then what is that feature there for???? I used it yesterday. If eBay don't like it then there are plenty of us who would just cancel the transaction and ask the buyer to purchase from our website where we have ALL of our items listed ..... but somehow I don't think eBay would want us to start doing that.

100% with you Austeam.
If a buyer asks us for a cost variation on an order they have placed through eBay we suggest to them that we will just add the extra to the invoice.
If they want something extra we give them the options of buying through eBay (if its not listed we give them the option of having it listed) or have us raise a PayPal invoice for them.
Their choice. It's not fee avoidance - once they have purchased off us through ebay (and ebay have their fees for that transaction) they are our customer for extra sales, not ebay's (unless they opt to go through eBay).
Conversely - if we decide to give a partial refund (eg if a buyer purchases several postage included items, which is something that happens frequently) there is no scope on ebay for us to reduce the price and get fees back. We just have to carry the fees.
It works both ways.
Personally, I'd like to see an option similar to the PayPal invoice that can be issued from WITHIN ebay (and which would incur fees). Whilst I don't like paying the fees there are a lot of buyers who feel more comfortable buying within the ebay envelope - I'm pretty sure we've lost sales because that wasn't an option.

Exactly tona, I made the suggest this year to ebay on having an add item to invoice from unsold or have a brief description plus amount.
And I guess ebay don't want sellers adding to purchases that buyer hasn't confirmed.
Yet it would be relatively easy on invoice to have -
Add item - Brief description - amount, then send invoice. (similar to different postage options in listing an item).
Before buyer pays, there is an accept or decline button on the additional item. Buyer can do either and pay. FVF charged as normal BIN.
