Am I right by saying that the customer has to request combined postage>
There is a paypal protection right? I mean to get paypal protect items must be sent individually.

For Paypal Seller protection all items paid for on the one invoice should be posted together.
If they are paid for separately then for Paypal Protection they need to be posted separately.
If they are paid for together - they are posted together.
A buyer can request a combined invoice or the seller can send one without the buyer asking.

You can use Australia Post's, Click & Send, service which enables you to combine items that have been
paid individually into one package and you still get full PayPay protection.

...or in our particular case we use large letter for 99% of our items and as such have no seller protection anyway since there is no tracking on standard mail. Also all our items are listed as "FREE" postage so we are unable to discount for combined postage anyway.
So in these cases we just choose if/when we will combine items together. If the order is over about $50 we will use C&S small satchel parcel. We even often combine across our two stores. We always send a message to the buyer advising them that their order(s) will be combined tho, if it is not paid in a single transaction.
For multiple orders we will often include a free item similar to what they have already ordered to compensate to some extent. I don't think we have ever received a thank you for doing that.
We have never had a formal INR claim against us. Just the occasional (about 1 in 1000) claim thru messaging. For those we just send out a replacement item after asking a few pertinent questions.

Again.....you have free postage on all your items.
There is no postage to be combined
If you have it incorporated in your item price, and someone buys
more than one, then they are obviously happy with the price

Unfortunately, that isn't actually the case. The click and send system will allow any items with matching address details to be combined into a single package (whereas eBay's system won't), but the PayPal seller protection policy expressly states that doing so disqualifies a seller from protection.
I do know that some sellers have successfully defended INR claims after combining items into a single package, but there's actually a difference between winning a dispute (or the buyer's claim not being successful) and qualifying for seller protection, since the latter is more about when a claim or chargeback is successful and the buyer's payment is refunded, but the seller isn't charged. In the case of a chargeback, separate proof of postage would be required for each payment.
To the OP - you can combine items on an invoice but you can't discount or reduce the postage or price because it's already at $0 when you use free postage.
If you want to encourage more buyers to make single payments for items up front, I would suggest maybe looking at using a promotional discount offer, because then your listings will display the promotion (which encourages buyers to use the cart to qualify). You can offer small discounts on the item prices, or bonus items (eg like a buy 3 get one 1 type of deal) if more than one item is purchased.
