A US buyer has purchased one of my items and I have no idea how that was possible as I post to Australia only.
What do I need to do to fix this please?
I've messaged the buyer to let them know that I don't post to the US.
The item item number is 252273442053,(so you can see that I post to Australia only).

I always thought that if you have all the blocks in place they simply can't purchase?
The block works on the primary delivery address - about half the OS buyers who make attempts to buy from me are IDs I actually recognise as buyers, but they've been able to buy as they changed their postage address on record with eBay to an AU delivery address. In those cases the block will allow them through, regardless of the registered address (I've done this myself with a US seller who only posted within the US, a very kind member of the forum allowed me to use an address over there and forwarded the item to me afterwards).

This buyer has a feedback score of 7 and they have bought 4 items in the last month.
The purchase history had a US address listed so maybe they may have changed that if they had purchased via Paypal?
I probably should've waited to hear from them and see why they were able to purchase and did they have and Australian
address to send it too.
I will just have to wait and see if they reply or if I have to close the dispute and hopefully it will all go smoothly.

To be honest I think I would of cancelled the sale asap to once a buyer pays I find they get more stubborn and if it turns out they do have an Aus address to post to they can always buy it again.

I have used the cancellation request: Your cancellation reason: The buyer asked to cancel the order, or there's an issue with the
buyer's address
